Ronin Combat League Fighter paperwork

Battle in the Boro · Sat Dec 5 2026

Get licensed to fight.

Every fighter on the card, pro or amateur, needs a Kentucky license from the Kentucky Boxing and Wrestling Commission (KBWC). You send your paperwork to KBWC yourself; Ronin doesn't collect it. Every form you need is on this page.

Apply on KBWC's eServices portal. You upload your physical and bloodwork there, so get those first.

Get it all to KBWC by Sunday That's Ronin's deadline, not KBWC's. It leaves time to fix a missing form or a failed test well before fight week.

Four steps

  1. Get a physical

Take KBWC's Physical Report form to your appointment. You fill in pages 1 and 2; the doctor does 3 and 4 and checks the "is medically fit" box. Have an MD or DO sign it: the form's first page says other signatures won't be accepted.

The physical can't be more than 90 days old when you apply, so do this first and apply soon after.

  2. Get your bloodwork

Ask the doctor to order these at your physical:

    • HIV antibody
    • Hepatitis B antigen
    • Hepatitis C antibody

Get a copy of the lab report to upload. The results have to be less than a year old on fight night (drawn on or after Dec 5 2025). A positive result means you can't compete.

  3. Apply online

First Kentucky license: start on the eServices home page. It asks for your name, date of birth and Social Security number, then walks you through the application. Upload your signed Physical Report and your bloodwork results there. Check Amateur MMA or Professional MMA.

Licensed with KBWC before: sign in (or create your account) and renew or apply from there.

Started and didn't finish? Enter your reference number on the home page to pick up where you left off. Call KBWC to confirm the license fee.

  4. Check your license

Look yourself up on KBWC's license search before fight week. Not there? Call them.

Saving and printing: the forms open in your phone's PDF viewer. iPhone: tap Share, then Save to Files or Print. Android: tap ⋮, then Download or Print. No printer? Your gym, a library, or a UPS Store can print them.

Can't apply online?

Fill out KBWC's MMA/Boxing Contestant Application in ink and mail or hand-deliver it with a copy of your government photo ID, the signed Physical Report, the original or a certified copy of your bloodwork, and the license fee by check or money order to "Kentucky State Treasurer". KBWC's office is closed weekends, so mailed paperwork has to arrive by Fri Nov 13. Allow for the mail.

Already licensed this year?

A KBWC license runs Jan 1 to Dec 31, so a 2026 license covers Dec 5. Check it on the license search, and make sure KBWC has bloodwork from after Dec 5 2025. If yours is older, get new tests and upload them on eServices.

Fight day

Coming back from a KO or injury

A knockout means at least 60 days out. To be cleared, your doctor's office faxes or mails KBWC the Medical Release Form; you don't send it yourself. KBWC may also ask for a neurological evaluation, done by a neurologist or neurosurgeon. Only use these forms if this applies to you.
